Step 5 of 7
Payment plan
First, let us know if this is your child's first time with Ground Up BJJ — this determines whether a kimono is included.
🥋
Kimono included as standard. Your child's kimono (BJJ gi) is included in the price and will be ready for them on the first day of class. It's theirs to keep.
🥋 Kimono sizing
So we can order the right size kimono for your child's first day. Heights and weights are approximate — we'll always size up if in doubt.

Participation waiver & liability release

I, the undersigned parent/guardian, give permission for my child to participate in Ground Up BJJ after-school classes operated by Ground Up BJJ Ireland. I understand that Brazilian Jiu Jitsu is a contact martial art and that, despite all reasonable precautions, there is an inherent risk of injury.

I confirm that my child is in good health and physically capable of participating. I have disclosed all relevant medical conditions, allergies, and medications in this enrolment form.

I agree to release, discharge, and hold harmless Ground Up BJJ Ireland, its coaches, employees, and affiliates from any and all liability, claims, demands, or causes of action arising out of or related to my child's participation in the programme, except in cases of gross negligence or wilful misconduct.

I consent to Ground Up BJJ coaches administering basic first aid in the event of an injury and authorise emergency medical treatment if my child is injured and I cannot be reached.

I consent to photographs or video of my child being taken during classes for use in Ground Up BJJ promotional materials, unless I notify Ground Up BJJ in writing that I do not consent to this.
